We are currently seeking a Licensed Vocational Nurse (LVN) to conduct home visits and provide essential nursing care to our clients in a comfortable, familiar environment.

Job Responsibilities:
? Conduct in-home patient assessments and monitor vital signs.
? Provide nursing care that includes medication administration and injections.
? Educate patients and families on disease management, treatment plans, and home care techniques.
? Coordinate with physicians, RNs, and other healthcare professionals to ensure quality care.
? Document patient progress and maintain accurate medical records.
? Ensure compliance with all state, federal, and company regulations.
? Travel to clients' homes within the designated service area.

Qualifications:
? Active LVN license in the state of Texas.
? Minimum 1 year of nursing experience (familiarity with the HCS Program preferred).
? Excellent communication and patient care skills.
? Ability to work independently and manage a flexible schedule.
? Valid driver’s license and reliable transportation required.
